To support a growing telehealth initiative, the full-time Rhode Island Licensed Telehealth Physician will collaborate with healthcare advocates to facilitate patient care, oversee care plans, and address social determinants of health in a remote setting.
Key responsibilities Learn and utilize Solace systems and tools while providing clinical expertise in patient interactions Identify and prioritize patients' needs to maintain a streamlined care continuum Oversee patient care plans in collaboration with healthcare advocates to address various social determinants of health Required qualifications Board certification in family medicine, internal medicine, or a relevant specialty DEA license and HAP certification Strong clinical skills with excellent organizational abilities Experience working with diverse patient populations, including elderly patients Technical proficiency and adaptability to current systems and software
