Job Description

- Provide ministry-funded, community-based occupational therapy services in clients’ homes and/or through virtual care
- Conduct occupational therapy assessments using interviews, standardized tools, and informal assessment techniques, including:
- Home safety and home modifications
- Equipment prescription
- Fall prevention
- Mobility and physical functioning
- Cognition
- Stroke rehabilitation and chronic conditions

- Develop and implement evidence-based intervention plans in collaboration with clients and caregivers
- Assign, supervise, and monitor occupational therapy assistants as appropriate
- Educate clients and caregivers and connect them with relevant community agencies and resources
- Complete funding applications when required
- Maintain accurate, timely, and skilled documentation, including:

- Assessment findings
- Care plans
- Progress and discharge summaries
- Reports aligned with referral source requirements and COTO standards
