This Part Time position serves patients at Providence Transitional Care Centre - Level 3. Working under full scope, you will provide direct care for defined, established needs and predictable outcomes. Your practice supports clinical transitions and quality of life through physical, emotional, social, and spiritual care.

Key Responsibilities:

- Provide direct care to assigned patients
- Assist with evaluating patient health status
- Share priorities for preventive and rehabilitative care
- Recognize patients and caregivers as decision makers
- Report care information to key stakeholders

Requirements:

- Unrestricted registration with the College of Nurses of Ontario
- Current CPR certification
- Required college certificate; diploma preferred
- Medication certificate after 1985 when required
- Seek assistance when care exceeds knowledge or scope
