Oxford University Hospitals NHS Foundation Trust is seeking a Paramedic for cooperative community work. You will help improve healthcare access for practices’ highest need patients and families. The role supports acute illness, chronic condition treatment, longer term frailty, end of life care, and hospital admission prevention. Daily GP clinical input supports your patient assessment, diagnosis, treatment, and referral decisions.

Key Responsibilities:

- Improve access for highest need patients
- Support patients and families during health crises
- Deliver care in patients’ own home environments




- Address individual complex health and social needs
- Help prevent hospital admission where appropriate

Requirements:

- Paramedic capability in community clinical care
- Ability to assess and diagnose patient needs
