This Field Nurse Case Manager role supports Patient Services across Atlantic Canada and Quebec. You will answer program questions, provide medical education, complete enrolment activities, and support ongoing patient adherence. The position also involves clinical monitoring, funding coordination, reimbursement assistance, and communication with physicians, pharmacies, and other care partners.

Key Responsibilities:

- Answer telephone and web-based questions about program services
- Support patient enrolment and physician documentation activities
- Provide telephone maintenance and adherence support




- Coordinate funding searches, coverage renewals, and reimbursement support
- Attend physician offices, patient meetings, and conferences as needed

Requirements:

- Registered Nurse licensed in positive standing within the respective province
- Two to four years of clinical experience
- Required bilingualism in English and French
- Ability to resolve conflict professionally
- Ability to work effectively in a team environment
