Apply mental health assessment and counselling expertise as a Social Worker Mental Health Clinician with the Iroquois Falls Family Health Team.

This clinician supports patients experiencing mental health and psychosocial concerns within an interdisciplinary primary care team. The work includes individual counselling, therapeutic interventions, individualized care planning, and connections to additional services. Key Responsibilities:

- Provide individual counselling and brief therapeutic interventions
- Develop individualized treatment plans and monitor patient progress

Requirements:

- Bachelor’s or Master’s degree in a related mental health discipline
