As a Health Navigator, you will work closely with the Health & Wellness Manager to coordinate resident assessments, care plans, and wellness services. The position involves collaboration with residents, families, physicians, and care teams. Your work supports personalized, high-quality care through nursing knowledge, documentation, and resident education.

Key Responsibilities:

- Complete resident assessments and individualized care plans
- Coordinate admissions, conferences, transfers, and discharges
- Provide nursing care and medication administration




- Partner with physicians, families, and care teams
- Monitor wellness outcomes and clinical documentation

Requirements:

- RPN, LPN, or RN registration in good standing
- Current CPR and First Aid certification
- Medication administration knowledge
- Healthcare experience in retirement or related settings
- Solid assessment, clinical, organizational, and communication skills
