Job Title Personal Support Worker – Multiple Locations (Aurora, Barrie, Bradford, King City, Newmarket, North York, Orillia, Penetanguishene, Stouffville, Toronto, York, ON) Job Summary Provide personal care and support to vulnerable, at‑risk older adults and seniors across multiple community living sites, ensuring safety, medication management, and social engagement in line with program values. Key Responsibilities Assist clients with physical, cognitive, mental‑health, and social‑recreational needs. Maintain accurate care documentation and support daily living activities. Respond to emergencies, document incidents, and ensure overall safety and security. Monitor medication administration and storage, coordinating with physicians, pharmacists, and program staff. Plan and lead social and recreational activities, including driving clients to outings. Perform operational duties such as coordinating emergency evacuations, maintaining daily residency operations, and managing communications. Work rotating shifts across sites, including evening, weekend, holiday, overnight, and daytime hours. Provide various delegated support tasks as directed by senior staff. Qualifications Diploma/certificate or higher in Human Services, Gerontology, Personal Support Worker, or a recognized college/university program (preferred). First Aid and CPR training.



Experience with clients who have special needs, including cognitive impairment and/or mental health issues. Knowledge of medication management, general health, and psychiatric conditions. Solid organizational and problem‑solving skills; ability to work with individuals and groups. Positive, supportive client interaction; teamwork with multi‑disciplinary professionals. Good oral and written English communication skills. Computer skills. Valid Ontario driver’s license (preferred). Ability to work rotating shifts and at client’s homes. Pass a satisfactory vulnerable sector check (VSC). Work Environment Physically demanding body mechanics such as lifting and transferring. Work location in client homes may expose staff to secondhand smoke. A car is not required, but driving for extended periods with frequent stops is essential.
