If you're a motivated RN who thrives independently and truly loves patient-centered care, we want to hear from you!Responsibilities:Conduct comprehensive patient assessments and develop individualized care plans for home health patientsAdminister medications, wound care, IV therapy, and other skilled nursing treatments in the home settingMonitor and evaluate patient health status, documenting changes and reporting to physicians as neededEducate patients and family members on disease management, medication adherence, and home safetyCoordinate care with interdisciplinary team members including physical therapists, social workers, and physiciansMaintain accurate and timely clinical documentation in compliance with Medicare, Medicaid, and agency standardsEnsure patient care aligns with OASIS assessments and home health plan of care requirementsRequirements:Active and unrestricted Registered Nurse (RN)
