Support patients and healthcare providers through specialized programs as a Nurse Case Manager with Sentrex Health Solutions. No travel is required.

This Patient Services position coordinates program delivery and helps patients access prescribed drug therapy. The Nurse Case Manager manages cases, answers program questions, provides disease and therapy education, and supports reimbursement activities. You will communicate with patients, physicians, pharmacies, insurers, and other healthcare specialists while following established protocols.

Key Responsibilities:
Complete patient enrolment, registration, and required follow-ups
Answer telephone and web-based program inquiries
Provide adherence support through inbound and outbound calls




Review patient information for accuracy and completeness
Advocate for coverage renewals and funding decisions

Requirements:
Registered Nurse licensed in positive standing provincially
Two to four years of clinical experience
Experience supporting Patient Support Programs is strongly valued
Immunology experience is considered an asset
English and French bilingual ability is an asset

Use nursing expertise, patient advocacy, and program coordination to help maintain therapy access and continuity.
