Lead Community Health Worker practice for Island Health's Port Alberni Home and Community Care Services as a practicing Licensed Practical Nurse.

This casual supervisor coordinates Home Support Services while supporting direct care in community health settings. You will collaborate with an interdisciplinary team, implement service plans, and evaluate CHW skills. Nursing duties follow BC College of Nurses and Midwives standards, employer policies, procedures, limits, and conditions.

Key Responsibilities:
Coordinate delivery of Home Support Services
Supervise CHW service and direct care practice
Collaborate with the interdisciplinary team
Implement service plans for community clients
Deliver personal and nursing care

Requirements:
Practicing LPN registration with BC College of Nurses and Midwives
Recent acute, residential, or community care experience
Three years related experience including one year supervising
Ability to establish rapport with clients
Ability to work independently and collaboratively
