The OakMed Family Health Team (OFHT) provides cooperative, interdisciplinary, comprehensive primary health care to a population of patients in the Oakville area. The FHT is committed to compassionate, patient-centered care emphasizing illness prevention, chronic disease management, harm reduction and health promotion. The RPN position is critical to the effective functioning of the clinic. This position requires excellent nursing skills, an ability to work in a fast-paced, changing environment and strong interpersonal skills to work with patients, interdisciplinary health care providers and physicians. The RPN assists with all levels of patient care and works to full scope of practice within our Family Health Team. Hours are 8:30am to 5:00pm. Experience in primary care, especially in a family doctor's office setting, is preferred.
