Support residents at HART Hub as a Weekend Case Worker using trauma-informed practice, relationship-building, and frontline stabilization support in Whitby.

Durham Community Health Centre seeks part-time contract Case Workers for Saturday and Sunday, 12 PM to 12 AM. You will build trusting relationships, support daily needs, identify concerns, and assist with individualized housing and stabilization plans. Collaboration with Case Management, Housing, Primary Care, Addictions, and System Navigation teams is central to coordinated support.

Key Responsibilities:

- Build trusting relationships and provide consistent resident support
- Welcome residents and explain program services and expectations
- Conduct check-ins and respond to identified needs




- Complete wellness checks and respond to urgent situations
- Maintain accurate documentation in HIFIS and PSS

Requirements:

- Post-secondary education in social services or related field
- Trauma-informed, person-centred, and non-judgmental approach
- Solid communication, observation, documentation, and problem-solving skills
- Current First Aid and CPR certification required
- Vulnerable Sector Screening required
