Reporting to the Director of Care, this LPN delivers goal-directed nursing care under provincial standards. The role combines assessments, care planning, medication administration, documentation, and interdisciplinary coordination. It also includes resident education, delegation, emergency response, and transitions.
Key Responsibilities:
- Deliver direct nursing care, medications, and treatments
- Maintain assessments, care plans, and accurate documentation
- Coordinate care with physicians, teams, and service providers
- Respond to condition changes and medical emergencies
- Support infection prevention and quality improvement initiatives
Requirements:
- Current provincial or state Licensed Practical Nurse registration
- Solid clinical knowledge and care coordination skills
- Current CPR and First Aid certification
- Electronic documentation and Microsoft Office proficiency
- Vulnerable Sector Check completed or obtainable
