Job Overview The Licensed Practical Nurse (LPN) provides attentive, compassionate patient care, supports treatment plans, and communicates important updates to the care team. This role helps patients feel secure and supported while enabling colleagues to coordinate consistent care. Duties
- Monitor patients’ conditions, document observations accurately, and report changes to the appropriate care team members.
- Provide nursing care, administer medications, and support pain management in accordance with established care plans and procedures.
- Assist with phlebotomy, laboratory tasks, and patient needs; use medical terminology and electronic records such as Epic or Cerner as applicable.
- Support patients across care settings, including senior care, dementia care, hospice, pediatrics, and home care,



as applicable to the role. Requirements Required: Practical nursing capabilities, sound patient-care judgment, accurate documentation, and clear communication are essential for safe care and effective collaboration. Preferred: Nursing home, hospital, acute care, medical office, assisted living, or home care experience; familiarity with dementia care, pediatrics, children, senior care, hospice, phlebotomy, laboratory work, medication administration, Epic, Cerner, medical terminology, pain management, or case management is advantageous.
