Fraser Health is seeking a Licensed Practical Nurse, Full Scope, for its Home Health program. You will independently manage an assigned client caseload while collaborating with interprofessional teams. The position focuses on chronic disease treatment, client education, care planning, and helping clients live safely at home.

Key Responsibilities:

- Screen referrals and complete initial and ongoing client assessments
- Develop shared care plans with clients, families, and providers
- Deliver clinical interventions and explain care comprehensively
- Facilitate care conferences, referrals, discharges,



and transfers
- Coordinate joint home visits and community resource advice

Requirements:

- Current practicing registration with BCCNM
- Recent experience assessing complex chronic health conditions
- Recent care and discharge planning experience
- Valid BC driver's license and access to personal vehicle
- Ability to prioritize diverse healthcare issues independently
