130010 Axium Extendicare LTC LP is seeking a Registered Practical Nurse to provide planned, patient-centred nursing care in a long-term care setting. You will administer medications, document actions, and participate in interdisciplinary rounds with the DOC/DHW to ensure protected transfers and care plan effectiveness.

You will coordinate nursing team assignments, respond to changes in resident condition, and engage with residents and families to support education and informed decisions.
