Scarborough Health Network is seeking a Nurse Practitioner to lead geriatric care in the GAIN Clinic, focusing on assessment and management of frail seniors. This role emphasizes autonomy and collaboration with an inter-qualified team to promote health and safety for adults and elderly patients.

The NP will conduct outreach consultations to LTCs, retirement homes, and community settings, including home visits, and provide liaison support to inpatient and ED teams for complex cases.
