Deliver occupational therapy in GTA home care, using assessment and treatment expertise to help older adults transition safely home. Work as an independent contractor.

As an Occupational Therapist with Bayshore Health Care’s ICS integrated care model, you will assess, treat, educate, and manage clients requiring occupational therapy services. You will support independence and meaningful outcomes within communities while collaborating through one interdisciplinary team and shared client record.

The work workplace is home care.

Key Responsibilities:
Complete comprehensive occupational therapy assessments
Develop individualized treatment plans, goals, and discharge recommendations
Evaluate and modify treatment plans as client needs change




Support hospital-to-home transitions through evidence-based interventions
Communicate findings, recommendations, and education to clients and families

Requirements:
Master’s degree in Occupational Therapy or recognized equivalent
Registration in positive standing with COTO
Solid clinical reasoning and decision-making abilities
Proficiency with electronic documentation and secure digital communication
Valid Ontario driver’s license and reliable transportation
