Horizon Health Network seeks a Patient Care Attendant 2 for acute care or family health team settings. The role includes patient-focused care, delegated interventions, data collection, and documentation. You will work within your specified scope under nursing direction and collaboration.
Key Responsibilities: Assist patients with feeding, bathing, elimination, mobility, transfers, and dressing
Perform skin, pain, safety, and proactive rounding checks
Record weights, heights, intake, output, and vital signs
Gather supplies, assist procedures, and perform delegated interventions




Escort patients, respond to call bells, and complete documentation
Requirements: High school graduation or GED equivalency
Personal Support Worker certificate or equivalent education and experience
CPR/BLS certification for health care professionals
Flexible availability for days, evenings, nights, weekends, and holidays
Ability to perform duties with a valuable work record
