Using a person-centered approach, you will develop a care plan supporting the client/family to identify needs, focus on strengths, set achievable personal health and self-care goals and provide direct nursing care.

In collaboration with the client/family you will ensure services are provided and modified on an ongoing basis, based on client’s response to interventions, incorporating stewardship into service authorization.

This position is with the Self Managed Care Team which is a specialty Team within Home Care.

This Team looks after the needs of our clients who choose to hire their own Caregivers.

Description: As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being.

In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach,



counsel and advocate on behalf of patients and their families.

You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance.

You will provide safe, quality patient and family centered care while reflecting the shared vision and values. PLEASE NOTE: Provisional Permit holders will be paid $ to $ per hour.

Upon obtaining a full practice permit, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article or (ii), as applicable.

Required Qualifications: Completion of an accredited nursing education program.

Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA).

Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification.

Additional Required Qualifications: Preferred Qualifications: Baccalaureate degree in Nursing.

Case mgmt exp strongly preferred.

InterRAI training, continuing care legislation, and decision making and capacity.

Exp with an adult pop aged 18–64, chronic disease management, palliative care, mental health, addictions, trauma informed care, interdisciplinary coordination.

Direct clinical care inc nursing interventions, med management, wounds, HPTP, ostomy, mobility, transfers, equipment, family assessment, independent community practice.
