Shape personalized home-care plans as a Case Manager LPN with Right at Home Canada Fraser Valley across Abbotsford and surrounding communities.
This Case Manager coordinates client care journeys for older adults and adults with disabilities. You will assess needs, design care plans, coordinate caregivers, and maintain communication with families. The position also supports office priorities, staffing coverage, community relationships, and service inquiries.
Key Responsibilities: Visit clients and write or update personalized care plans
Select caregivers based on skills, experience, and availability
Guide caregivers through care tasks and follow up
Monitor clients, families, caregivers, records, and concerns
Review care hours and coordinate coverage changes
Requirements: Current practising LPN registration with BCCNM or equivalent
Experience caring for older adults
Solid organization, communication, and follow-through
Comfort using Microsoft Office and electronic care records
Valid licence, reliable vehicle, and current criminal record check
