Your Opportunity: At the Silver Willow Lodge in Nanton, we provide personal and professional care services in Type B Continuing Care for clients with needs that fall within the psychosocial, physiological, health related behavior, and environmental domains.

We are recruiting a Registered Nurse in a role of a Case Manager to coordinate the placement, assessments of on-going referral needs, care planning (both complex and annual), and delivery of skilled and personal care services to our clients.

Note- Nanton is approximately 1 hour South of Calgary Description: As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being.

In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families.

You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance.

You will provide safe, quality patient and family centered care while reflecting the shared vision and values. PLEASE NOTE:



Provisional Permit holders will be paid $ to $ per hour.

Upon obtaining a full practice permit, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article or (ii), as applicable.

Required Qualifications: Completion of an accredited nursing education program.

Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA).

Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification.

Additional Required Qualifications: Preferred Qualifications: Baccalaureate degree in nursing is preferred; however, skills and experience relevant to nursing practice in community sector will be considered.

Recent community or continuing care related experience with the Integrated Supportive and Facility Living team preferred.

Geriatric and dementia experience/certification is an asset.

Dementia Care Training is an asset.

Palliative experience is an asset.

Experience with Connect Care and Netcare systems as well as interRAI experience.
