This temporary full time Care Coordinator position serves Ontario Health atHome’s Community program through the Scarborough branch. You will assess eligibility, develop care plans, connect patients with providers, and monitor service delivery. The work involves patients, families, caregivers, and health care practitioners.
Key Responsibilities: Assess care needs with patients and families
Determine eligibility and develop individual care plans
Link patients with community service providers
Coordinate and monitor care plan delivery
Respect patient values, preferences, needs, and choices
Requirements: Current registration with an applicable Ontario regulatory body
At least two years’ recent community health experience
Knowledge of health care systems and community resources
Strong assessment, communication, and decision-making skills
Ontario driver’s licence and reliable vehicle access
Bring registered health professional expertise, care planning, and community coordination skills to Ontario Health atHome patients.
