Reporting to the Director of Care, this LPN delivers goal-directed nursing care under provincial standards. The role combines assessments, care planning, medication administration, documentation, and interdisciplinary coordination. It also includes resident education, delegation, emergency response, and transitions.
Key Responsibilities: Deliver direct nursing care, medications, and treatments
Maintain assessments, care plans, and accurate documentation
Coordinate care with physicians, teams, and service providers
Respond to condition changes and medical emergencies




Support infection prevention and quality improvement initiatives
Requirements: Current provincial or state Licensed Practical Nurse registration
Robust clinical knowledge and care coordination skills
Current CPR and First Aid certification
Electronic documentation and Microsoft Office proficiency
Vulnerable Sector Check completed or obtainable
