Key Responsibilities: Lead PATH and PACT clinical operations and program services
Support safe, timely transitions across care settings

Requirements: Undergraduate degree in health sciences or related field
Seven years’ recent related experience, including two leadership years
Experience leading and developing high-performing interdisciplinary teams
Excellent communication, organizational, and prioritization abilities
Knowledge of budgets, collective agreements, and clinical program planning
