Job Summary
We are seeking a dedicated and experienced Nurse Practitioner to join our healthcare team. The ideal candidate will provide comprehensive patient care across various settings, including primary care, senior care, and specialized medical services. This role requires a compassionate professional with solid clinical skills, capable of managing diverse patient needs, including pediatrics, dementia care, and acute conditions. The Nurse Practitioner will collaborate with physicians and other healthcare providers to deliver high-quality, patient-centered care while utilizing advanced medical knowledge and technology.

Responsibilities

- Conduct thorough patient assessments, including history-taking and physical examinations
- Diagnose and manage acute and chronic illnesses across diverse patient populations
- Prescribe medications and treatment plans in accordance with state regulations
- Administer injections, perform phlebotomy, and manage medication administration
- Develop and implement individualized care plans for patients, including pain management and case management strategies
- Utilize electronic health records systems such as Epic and Cerner for documentation and order entry
- Provide patient education on health maintenance, disease prevention, and medication usage
- Collaborate with multidisciplinary teams in nursing homes, assisted living facilities, hospitals, or home care settings




- Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes
- Maintain compliance with healthcare regulations and standards of practice

Requirements

- Valid Nurse Practitioner license in the state of practice
- Experience working with children, seniors, or in hospital settings preferred
- Strong knowledge of medical terminology, laboratory procedures, and medical office operations
- Skills in pain management, dementia care, home care, and acute care are highly desirable
- Experience with electronic health records such as Epic or Cerner is a plus
- Ability to perform patient care procedures including medication administration and phlebotomy
- Excellent communication skills with the ability to educate patients effectively
- Compassionate approach to patient care with a focus on quality outcomes
- Prior experience in nursing home, assisted living, or hospital environments is advantageous
