Part time Occupational therapist needed to service the Durham Region. Pay is $66 per visit NOT per hour.

This position covers all of the Durham Region including but not limited to: Ajax, Whitby, Oshawa, Bowmanville, Courtice, Uxbridge, Cannington, Beaverton, Sunderland and surrounding areas . Each visit is 1 hour long and then you go to your next client home. We do pay 40 cents/km between clients for mileage and clients are grouped together as much as possible to minimize travel between.

This is a part time position with a versatile day time frame of 8am-4pm or 9am-5pm.

Ministry funded, community based OT services provided in client homes. Position includes client OT assessments, care plans creation in collaboration with client and caregivers.

Must be registered and in good standing with the College of Occupational Therapists of Ontario (COTO).

As this is community visiting position a drivers licence (G or G2), a reliable vehicle and car insurance in your name are needed to perform this job.

Pay rate is $66/visit

Part time staff receive a $25 monthly phone allowance

Requirements:

Vulnerable Sector Police Check not over 12 months old

PSW certificate or international nursing degree





Valid CPR certificate

TB test results from within 12 months or chest xray from within 24 months

G or G2 drivers licence, a vehicle and insurance in your name

Must own a cell phone with data

Responsibilities:

Home Aafety/Modifications, Equipment Prescription, Fall Prevention, Mobility and Physical Functioning, Cognition, Stroke Rehabilitation and Chronic Conditions.

Client/Caregiver Education, connecting them with community resources where appropriate.

Accurate and timely documentation including: Assessment Findings, Care Plans as well as Progress and Discharge Summaries.
