Key Responsibilities:

- Monitor patients remotely using telemonitoring technology
- Provide reassurance, support, and assistance through remote communication
- Document patient observations and interactions accurately
- Escalate critical situations to the healthcare team




- Manage conflicts and identify changes in patient condition

Requirements:
- High school diploma
- Qualified English and French reading and writing proficiency
- Microsoft Office and technology platform proficiency
- Strong communication, observation, and problem-solving skills
- Flexibility for twelve-hour days, nights, weekends, and holidays
