Support seniors’ health at AgeCare Dawson through goal-directed nursing care, care planning, and treatment administration. Work in Chestermere on a regular part-time basis.
This experienced Licensed Practical Nurse works with an inter-professional team under the Director of Care. You will apply AgeCare’s person-centered Professional Practice Model. The role includes evaluating care plans, communicating changes, and coordinating resident conferences.
Key Responsibilities:
- Apply the person-centered approach within inter-professional practice
- Maintain current resident care/service plans and records
- Communicate assessment and care changes as required
- Assist with resident care conferences and participate where applicable
- Document care according to regulatory standards and policies
Requirements:
- Practical Nurse Diploma
- Current registration in the applicable province
- Membership in valuable standing with the applicable province
- Three-plus years supporting seniors’ health
- Current CPR training and computer proficiency
Use senior care experience, care planning, and clinical documentation skills to support AgeCare residents and their families.
