Key Responsibilities:
- Deliver personalized services using a holistic resident-centric approach
- Build trusted relationships with residents and families
- Evaluate care-plan effectiveness and make required changes
- Maintain accurate resident assessments, care plans, and documentation
- Coach and mentor unregulated care providers
Requirements:
- Current Registered Practical Nurse registration in the applicable province
- Education or experience in gerontology, rehabilitation, or long-term care
- Robust drive and understanding of role demands
- Teamwork and customer service focus
- Current Vulnerable Sector Check or willingness to obtain one
