You will complete assessments, develop care plans, implement and evaluate care, and connect clients with community supports. The position requires skilled judgment and collaboration with care team members during transitions.

Key Responsibilities:

- Complete comprehensive client assessments
- Develop individualized care plans using best practices
- Implement and evaluate client care
- Lead care for designated primary-provider caseloads

Requirements:

- Current College of Nurses of Ontario registration
- Current CPR certificate
