Winnipeg Regional Health Authority (WRHA) is seeking a Hospital Based Case Coordinator to assess eligibility for Home Care supports and develop care plans. Working with the client, family, and interdisciplinary team, you will coordinate discharge planning and determine if care is best provided in a personal care home or alternate environment.

The role requires RN certification (CRNM) and substantial nursing experience, with solid communication skills and proficiency in Procura and interRAI-HC
