Support primary care patients as a Registered Nurse with UHN, using comprehensive assessment, triage, and chronic disease management skills.

The Toronto Western Family Health Team seeks an RN for a temporary full-time contract. You will support diverse patients across all life stages through holistic nursing, health promotion, and education. Collaboration with families and interdisciplinary providers is central to this primary care work.

Key Responsibilities:

- Complete comprehensive nursing assessments considering social determinants of health
- Triage in-person and telephone patient requests for appropriate care
- Provide general primary care nursing according to standards
- Develop treatment plans with patients and interdisciplinary team members
- Deliver health teaching, counseling, and illness prevention

Requirements:

- Bachelor of Science in Nursing degree
- Current College of Nurses of Ontario registration
- Basic Cardiac Life Support certification
- Three to five years primary care nursing experience
- Availability for wide-ranging shifts and Saturday work
