VHA Home HealthCare seeks a Registered Nurse or Registered Practical Nurse for its Child and Family Program. You will deliver individualized, one-on-one care to children with complex medical needs in homes and other community settings. The position follows College of Nurses of Ontario standards, VHA policies, program requirements, and Community Health Nursing standards.

Key Responsibilities:

- Assess clients and coordinate individualized nursing care plans
- Reassess interventions and evaluate care plan outcomes
- Deliver safe, quality care to children and families




- Document care according to College of Nurses standards
- Report condition changes and participate in on-call rotation

Requirements:

- Registered in good standing with the College of Nurses
- RN degree or RPN diploma from recognized institution
- Valid First Aid or CPR certification
- Ontario G2 or G license and reliable vehicle
- Effective communication, documentation, and critical thinking skills
