Deliver evidence-based physiotherapy in clients’ homes and community settings, focusing on assessment, rehabilitation, mobility, and independence. Support aging in place through client-centred care.

As a Physiotherapist on the interdisciplinary Community Home Care Team, you will assess and treat mobility limitations, chronic conditions, injuries, pain, balance impairments, and fall risks. You will create individualized plans using therapeutic exercise, manual therapy, mobility training, and functional rehabilitation. Collaboration with healthcare providers supports integrated care and informed recommendations.

Key Responsibilities:

- Conduct comprehensive assessments of mobility, strength, balance, gait, and function
- Develop, monitor, and modify individualized treatment plans
- Provide therapeutic exercise, manual therapy,



and mobility training
- Implement fall prevention strategies and safe ambulation recommendations
- Document progress and communicate recommendations to the interdisciplinary team

Requirements:

- Degree in Physiotherapy from an accredited university program
- Current Ontario registration in valuable standing
- Current CPR certification and valid G licence
- Minimum three years of physiotherapy experience preferred
- Reliable vehicle and community travel availability required
