Lead clinical assessments and care planning as Nurse Manager with Qualicare Burlington across Burlington and Oakville. This is a full-time salaried leadership position.

As an RPN or RN, you will serve as the clinical backbone of Qualicare Burlington’s home care model. You will personally assess new clients, build care plans, and guide complex decisions. Your work supports families and establishes standards for the care team.

Key Responsibilities:

- Conduct every Aging Well Advisory assessment
- Build care plans for new clients
- Lead complex dementia and palliative cases
- Review every active client plan quarterly
- Provide clinical oversight to the care team

Requirements:

- RPN or RN in positive standing with the CNO
- Community or home care experience
- Confidence making and documenting clinical decisions
- Trusted communication with families during difficult conversations
- Belief that early engagement prevents harm
