General Site, 4001 Leslie Street#### Position SummaryPharmacist, Pharmacy ServicesYour goal in this position is to provide direct patient care to identify, solve, and prevent drug therapy problems based on an evidence-informed practice. You will also participate in patient education, support the teaching of students, provide drug information and/or education to nursing and medical staff, and ensure patient medication safety by supporting drug distribution services. You`re also a great relationship builder, communicate effectively with peers across the entire organization and effortlessly connect with physicians, staff, patients, and families in our diverse community. You are highly organized, have great interpersonal skills, demonstrate critical thinking and reasoning skills and adapt well in a dynamic environment. You will take on the shared responsibility for ensuring that health and safety policies are followed to provide for a healthy environment for patients, staff, physicians, volunteers and families.#### Qualifications
* Graduate from an accredited university Pharmacy Program
* Licensed under Part A of the Ontario College of Pharmacists
* Hospital residency or hospital direct patient care experience an asset
* Knowledge of pharmaceutical care process, disease management and clinical pharmacology
* Familiarity with evidence-informed practice
* Demonstrated skills in computerized pharmacy systems####

This is a Full Time Temporary position of 12 months with 8 hour shifts, Days, Evenings shifts including Weekends and On-Call in the Pharmacy Services Department.
