CHEO in Ottawa seeks a Social Worker to join an interprofessional palliative care team at CHEO, Roger Neilson House and in the community. You will provide counseling to children/youth and families facing progressive illnesses and support them through illness trajectories including end of life, coordinating resources as needed.

You will conduct psychosocial assessments, offer ongoing individual/family/group counseling, advocate for services, and collaborate with CHEO colleagues to ensure seamless
