Adult outpatient mental health and addictions care anchors this temporary full time Social Worker position at Humber River Health. Work occurs at the Wilson Site with cross-site coverage possible.

Within a Stepped Care Model, the Social Worker delivers individual and group-based care for acute stabilization and short-term episodes. The position connects patients with community resources after discharge and addresses mental health, addiction, and concurrent disorders when applicable. Practice requires CBT certification, DBT skills, motivational interviewing, and Trauma Informed Care experience.

Key Responsibilities:

- Screen referrals and complete detailed risk assessments
- Conduct psycho-social assessments using holistic approaches
- Apply educational, supportive, CBT, DBT, and solution-focused modalities




- Coordinate treatment-team discharge plans and referrals
- Provide discipline-specific expertise to treatment-team members

Requirements:

- MSW from an accredited School of Social Work
- Two years’ experience in mental health or addictions
- Individual and group counselling skills
- Experience applying DBT skills and motivational interviewing
- Knowledge of community resources and crisis response

Support adult mental health and addictions patients through assessment, counselling, therapeutic skills, and community discharge connections.
