Acclaim Health seeks an established part time Registered Practical Nurse for its Oakville Community visiting team. Provide nursing care in clients' homes, clinics, and the community while supporting independence, safety, and well-being. The work includes health assessments, palliative care, chronic disease management, and collaboration with an interdisciplinary team.
Key Responsibilities: Provide nursing care within community and clinic settings
Assess client health status and organize daily priorities
Develop, evaluate, and modify plans of care
Administer IV therapy, medications,



and injections
Provide wound and catheter care and maintenance
Requirements: Current registration with the CNO
Current CPR certificate and medication administration ability
One to two years' community clinical practice preferred
Valid driver's license and access to a vehicle
Excellent interpersonal, communication, and teaching skills
Bring reliable community nursing, assessment, medication administration, and wound care expertise to Acclaim Health clients in Oakville.
