Key Responsibilities: Deliver personalized services using a holistic resident-centric approach
Build trusted relationships with residents and families
Evaluate care-plan effectiveness and make required changes
Maintain accurate resident assessments, care plans, and documentation
Coach and mentor unregulated care providers
Requirements: Current Registered Practical Nurse registration in the applicable province
Education or experience in gerontology, rehabilitation, or long-term care
Solid drive and understanding of role demands
Teamwork and customer service focus
Current Vulnerable Sector Check or willingness to obtain one
