Support residents overnight at HART Hub in Whitby using trauma-informed practice, relationship-building, and calm crisis response within a 24/7 stabilization team.
Durham Community Health Centre seeks Case Workers for two full time contract overnight vacancies. The 35-hour Monday-to-Friday schedule supports residents experiencing homelessness or housing instability. You will provide frontline support, develop trusting relationships, and coordinate practical assistance with HART Hub teams and community partners.
Key Responsibilities: Build trusting relationships and provide consistent frontline resident support
Conduct wellness checks and respond to concerns or urgent situations
Support individualized stabilization plans, housing tasks, and referrals
Maintain safe, welcoming, organized stabilization units




Complete accurate documentation and communicate across shifts
Requirements: Post-secondary education in social services or related field
Relevant education or equivalent experience may be considered
Experience with homelessness or complex social barriers is an asset
Current First Aid and CPR certification, or timely completion
Vulnerable Sector Screening and reliable transportation may be required
Bring compassionate engagement, objective documentation, and dependable overnight support to residents at Durham Community Health Centre’s HART Hub.
