Build a new home care program in Scarborough through comprehensive physiotherapy, mobility assessments, and individualized treatment plans for clients at home.
This Community Physiotherapist role supports older adults and people with chronic or complex conditions. You will help shape service delivery while treating clients in homes and community settings. Collaboration includes Occupational Therapists, Registered Nurses, and Personal Support Workers through shared care plans and case conferences.
Key Responsibilities: Assess mobility, strength, balance, and independence
Develop individualized physiotherapy treatment plans
Provide falls risk assessments and balance training
Educate clients and caregivers on mobility and injury prevention
Document care and contribute to quality improvement
Requirements: Accredited university degree in Physiotherapy
Ontario registration in positive standing
Current CPR certification
Ontario G licence and reliable vehicle
Approximately three years of experience preferred
