Coordinate patient flow and discharge planning at Peace Arch Hospital in White Rock, B.C., supporting secure, organized ER operations.

This relief part time Patient Care Coordinator role supports Fraser Health’s ER team through clinical supervision and daily operational coordination. You will guide nursing and support staff, prioritize admissions, review care plans, and help patients progress toward discharge. The position is available until approximately October 2027 or the incumbent’s return.

Key Responsibilities:

- Plan patient discharge and appropriate placement or referral
- Review admissions, discharge planning, and individual care plans
- Adjust assignments, schedules, leave, and staffing coverage
- Address patient and family concerns and complaints




- Support quality improvement, safety audits, and risk management

Requirements:

- Current Registered Nurse registration with BCCNM
- Three years of recent designated-area clinical experience
- One year of supervisory experience or equivalent education
- Postgraduate coursework or certification applicable to the clinical area
- Ability to organize work, communicate, and operate related software
