As a School Nurse and Community Visiting Nurse, you will care for pediatric patients in schools and patients of all ages during community visits. You will support differing care complexities while helping patients remain comfortable at home. The work is located in the County Area, Erie St. Clair District.

Key Responsibilities
Assess individualized patient needs using appropriate tools and techniques Execute and continuously monitor individualized care plans Build rapport and trust through collaborative patient engagement Maintain and update professional patient encounter records Provide required after-hours on-call coverage for patient safety

Requirements:
Registered Practical Nurse registration with the CNO in valuable standing Valid driver's license and access to a vehicle Pediatric care experience is an asset Community care delivery experience is an asset Willingness to travel within a designated area Bring assessment, care planning, and patient relationship skills to ParaMed's school and community nursing services. #J-18808-Ljbffr
