This Community Care Manager connects clinical services with the wider community. The work combines in-home assessments, care planning, family onboarding, and outreach. You will build healthcare partnerships across Guelph, Cambridge, and Waterloo Region while guiding families through private care options.

Key Responsibilities:

- Evaluate physical, cognitive, and social needs in clients’ homes
- Draft comprehensive, person-centred care plans
- Guide families through intake and warm introductions
- Conduct reassessments and follow-up visits
- Visit healthcare organizations and attend community events

Requirements:

- Registered Practical Nurse or Registered Nurse in positive standing
- Registration with the College of Nurses of Ontario
- Local healthcare or home care system experience
- Relationship-driven communication with families and professionals
- Valid G-class license and reliable vehicle
