This casual Primary Health Care Nurse role supports comprehensive, coordinated care for clients. You will work autonomously while collaborating with inter-skilled teams, Primary Care Physicians, and Nurse Practitioners. Care may serve adults, seniors, pediatrics, and people with multiple chronic conditions.

Key Responsibilities
Assess clients in primary care, community, or home settings Plan, implement, and evaluate nursing care delivery Provide health promotion, prevention, and treatment services Monitor clients and support ongoing self-management follow-up Conduct integrated care planning and clinic visits

Requirements:
