Support geriatric and frail seniors at West Park Healthcare Centre as a Registered Nurse, using comprehensive assessment and rehabilitation nursing expertise. UHN seeks a permanent full-time Registered Nurse for the Geriatric Functional Enhancement Service and Frail Seniors Transition Home. You will coordinate nursing care within an inter-professional, primary nursing model, supporting health promotion, treatment, and optimal function. Days and nights are required.

Requirements:
