Provide psychosocial care at Scarborough Health Network as a hospital Social Worker with an MSW, serving diverse patients across Medicine CP4.

Reporting to the assigned unit Manager, this permanent full time position supports individuals and families throughout hospital clinical areas. The Social Worker delivers care and service while applying crisis intervention, psychosocial assessment, discharge planning, and community-resource knowledge. Work occurs across all shifts at the General Site.

Key Responsibilities:

- Provide care and service to diverse patient populations
- Support individuals and families across clinical departments
- Complete psychosocial assessments and discharge planning
- Link patients with appropriate community resources




- Apply crisis intervention skills during patient care

Requirements:

- Master of Social Work degree
- Registration with Ontario College of Social Workers
- Experience working in a hospital setting
- Excellent assessment, communication, and interpersonal skills
- Flexibility, sound judgement, and interdisciplinary collaboration
